- Birth Certificate in Security Paper (NSO) or BC issued by the Local Civil Registrar duly authenticated by NSO
- Four (4) colored passport size photos in royal blue background, decent attire w/ collar and sleeves
- Proof of identity / supporting documents indicating full name, date and place of birth, and citizenship
- Signature in checklist of documents submitted.
Additional Requirements If:
- MARRIED ~ Marriage Contract authenticated by NSO
- LATE REGISTERED ~ Baptismal Certificate
~ Voter's ID and affidavit or CTC of voter's affidavit
~ Any old public documents indicating birth particular and citizenship (old ID's)
Renewal

- Notarized affidavit of loss and police report to the Consular Records Division of the consular Affairs of DFA

- Birth Certificate in Security Paper (NSO) or BC issued by the Local Civil Registrar duly authenticated by NSO
- Four (4) colored passport size photos in royal blue background, decent attire w/ collar and sleeves
- Proof of identity / supporting documents indicating full name, date and place of birth, and citizenship

- Affidavit of support and consent signed by either parents w/ parent's passports and/or
- Affidavit of support and consent signed by the parent who is not traveling with ID/passport and/or
- If both parents are abroad, the affidavit of support and consent should be authenticated at the nearest Philippine Embassy or Consulate General and/or
- Photocopy of passport of the person travelling with the minor and/or
- Minors born after 03 August 1988, submit original/CTC of parents's marriage contract

Seaman
